Number 14. Dated Entradám, 3 March 1904.
Informant: Abrahám Munel, town crier, Entradám, whom the below-written assistant registrar knows personally.
Father: Mózes Werczberger, Jewish, merchant, residence and birthplace Entradám, age 35 years.
Mother: Mrs. Mózes Werczberger born Háni (Chaje) Mendel, Jewish, birthplace and residence Entradám, age 30 years.
Birth: Entradám, October 1850.
Child: boy, Jewish, Lázár.
Remark: The day and hour of birth are unknown. The Naszód district chief administrator gave permission with decree number 4055 dated 17 December 1903 for herein delayed registration of birth. For entry of data pertaining to the parents, the date of birth was taken as authoritative. Informant declares that he bears direct knowledge of the birth and was entrusted with making the report by Lázár Werczberger.
Read forth, explained to the non-Hungarian-speaking informant in the German language understood by him, approved, and supplied with the hand-mark of the informant due to his illiteracy.
