It's the sort of handwriting where everything could be at least two different things and you need a pretty good idea of what it says in order to read what it says, and the informant's given name just isn't slotting in anywhere for me. (I'm confident in my reading of the rest of it.)
(Don't worry about column headers. It's a civil registration. I know them by heart even in my sleep, and even if that weren't true, it's very easy to look them up.)
