Hello,
Here is all the relevant information. Note that Romania used the Julian calendar until 1919, so I’ve included the dates in the modern (Gregorian) calendar in parentheses. (This is a copy issued in 1912.)

Registration of Birth: Iași, 1 April (Gregorian: 13 April) 1886, 11 a.m. [11:00]

Child: Șmil

Sex: Male

Birth: Iași at strada Târgușor Nicolina No. 15 (the parents’ residence); 30 March (11 April) 1886, 4 p.m. [16:00]

Father: Avram Leib PERLMUTER, 41 years old, innkeeper, Jewish

Mother: Sura Malca PERLMUTER, 38 years old

The parents reside in division II of Iași.

Declarant: Father

Witnesses: Solomon HAIMOVICI, 67 years old, without profession, residing in division IV; and Alter CRAMER, 62 years old, without profession, residing in division II
