Please include the child's name; both parents' names and patronymics; surname spelling; father's status and registered town; place of birth; birth and circumcision dates, including Hebrew dates; and the circumciser's name. Please also transcribe and explain any marginal or later annotations.

I am researching the Gamolsky/Gomolsky family. Please mark uncertain letters or words rather than guessing. A partial reading is welcome. Please focus on entry 40, not the neighboring entries.
