This is a Passport application of Sora Leia Birger, from Grodno. It includes her baby daughter Dina and Sister in law Brayna. Can someone please translate the whole document and kindly check to see if Sora Leah's parent's names, both first and last are mentioned? Also please let me know if her home address is also mentioned. Thank you.

Представляя при этом установленные документы Покорнейше, честь имею просить Вашего Превосходительства выдать мне, имеющей от роду 22 лет и дочери моей Дине 1-го года и сестры мужа моего Брайны Симхелевны Биргер, имеющей 20 лет, с согласием отца её, шестимесячный заграничный паспорт на проезд нам за границу, и таковой соизволить выслать мне его по принадлежности согласно ниже написанного адреса моего в чём собственноручно и расписываюсь.

На выезд дочери моей Брайны за границу с женой сына моего изъявляю своё полное согласие.

(From) Sora-Leia Abramovna Itskovna Birger, a Grodno townswoman Petition.

Submitting herewith the required documents, I have the honor to most humbly request Your Excellency to issue a six-month foreign travel passport—enabling us to travel abroad—to myself (aged 22), my daughter Dina (aged 1), and my husband’s sister Braina Simkhelevna Birger (aged 20, with her father’s consent); and to kindly send said passport to me at the address written below, which I hereby confirm with my own signature.
Sora Leia Birger.
I hereby express my full consent for my daughter Braina to travel abroad with my son’s wife.

My address: City of Grodno, 1st District. Via the City Police Administration. Bolshaya Troitskaya Street, Andikovsky’s house.
