Please translate the Yiddish on this sheet. The Latin-script portions I can read; the Yiddish I cannot. Two blocks of Yiddish prose sit below the register section, four lines each, plus Yiddish beside the Latin entries and a line at top right. I would like all of it, but the two prose blocks matter most.

One thing is especially interesting. In brown ink, a cursive paragraph in the same hand and ink as a letter on the reverse of this sheet, dated 27 February 1941 from Naishtot. Beneath it sits a single isolated word, where a signature would go. That letter names nobody, not its writer and not its recipient. If you can read that one word, it would tell my family who wrote to them four months before the Germans came.

The Latin dates read 1907, but those are birth dates; the sheet was written in 1941 or later.

I would rather not say what I think it says, as three programs gave three different answers. If a word is illegible, please mark it illegible rather than guess.

My name.
{Here the same information is given in Yiddish as in Lithuanian. Above "Leibas Todresas" and "Sepe-Leibas" it says "father", and next to "Chane-Roche" it says "mother". Next to the dates it says "born". Above "Goldbergaite" it says "maiden name", and above "Zusmanoviciene" it says "current name". Below her birth date, it says "month" below "IV" and "day" below "i2"}

Note. My father's family name is not Zusmanovits but Leivi (Levi). I am called Zusmanovits because I was adopted into a family that is called Zusmanovits.
The adoption is legal; it was done by a court.
