Can you translate?

This is one of four images. This one consists of birth and death entries of Moses Jaged, one of my great-great grandfather's brothers who died as an infant. Separately, I'm posting birth and death entries of Michael Jaged, who also died in childbirth, and the birth entries from my great great grandfather Hersch Jaged and his brothers, Aaron and Isaak (the ones who lived, but I have no subsequent info). I'm also uploading the chart of the summary findings with links.

Can you translate from the images?

It's from Narajow, then Austria-Hungary, then Poland, then USSR, now Ukraine. The column headings are in German and Polish, but I think the handwritten info is Polish.
