Information Question

I would like a translation from Polish of the headings and categories of this document which appears to be a list of synagogue payments for the town of Grzymalow, Galicia, a province of the Austro-Hungarian Empire. The town was owned by a Polish Magnate Landowner, which may be why the headings are in Polish, even though German was the language of Austria. The inventory cover page is written in Latin. Besides the translation, any insights would be appreciated. I've only included two sample names on this image, but there are many townspeople, men and woman, including those who seem to be taking responsibility for the debts of others, on this list.
