This is the death record of my cousin Michaelis SCHLAMM.

He died during WW I as a soldier fighting for Germany, so I am particularly interested in finding out if the document provides details of where and how he died.

Please provide a translation from the German.

Before the undersigned employee of the civil record did attend today the known person, by his passport, the commercial assistant Bernd Schlamm, residing in Eberswalderstrasse 26 Berlin, and declared that Auguste Schlamm, born Rosenberg, 68 years old, of mosaic religion, residing in Berlin, Eberswalderstrasse 26, born in Mosche? , canton of Schimm?, widow of the merchant Hermann Schlamm, about whose origin nothing is known, deceased in Berlin, in the Eberswalderstrasse 26 in his presence on February 15 1920 in the afternoon at 9.30 o’clock.
