9/21/2020 5:40 AM Based on the permit of the head of the T-ínren (?) township No 5417 December 30, 1903 posteriorly registerd that the father legal is Lun or David Majer Sámuel, Jewish, age unknown, butcher; earlier resided in Salánk, now lives in Amerika. The mother Michlovics Zseni / Záli Jewish housekeeper, born in Csangova (?) earlier resided in Salánk, now in Tákos. The site of the birth is Salánk, time: 1880 (also written the Hungarian word of the number) year; months, day and hour not known.Gender of the child is boy, name Mór, religion Jewish. Salánk, 1903. December 31. X register
9/26/2020 11:36 AM It's not a name change, but a delayed birth registration.

Based on permit number 5417 dated 30 December 1903 of the chief administrator of the Cis-Tisza district it is post-registered that: the legal father Májer Sámuel Lun or Dávid, Jewish, butcher of unknown age, previously resident of Salánk, now of America; the mother Zseni (Záli) Michlovics, Jewish, homemaker, born in Csongova, previously resident of Salánk, now of Tákos. Place of birth: Salánk, date: 1880 one thousand eight hundred and eighty, month, day, and time unknown. Child's sex: boy, given name Mór, religion: Jewish. Dated in Salánk, 31 December 1903.

Salánk and Kis- and Nagy-Csongova were in Ugocsa county and are now in Ukraine (https://web.archive.org/web/20140511185811/http://www.radixhub.com/radixhub/gazetteers/1913/ugocsa.htm#127350). Tákos was in Bereg county and is now in Szabolcs-Szatmár-Bereg county, Hungary.
