This letter is summary of witness deposition of my mother in law Irena Neuman nee Erenreich. It is regarding acts of courage of Jadwiga Kozlowska is righteous gentile who hid my mother in-law in Krakow during Holocaust from 1943-1945.

As a participant. As a Jew, I was in hiding in the Aryan district of Cracow, [and am] conscious of the responsibility of being truthful in the facts given by me, below.

I state

Citizen Jadwiga Kozłowska-Borkowska, daughter of Paweł, born on the 9th of June 1918 in Skawina, was known personally to me since 1935.

The abovementioned [person] had a pseudonym - and fulfilled a function - she took part (this part describes: the record of service and activity in the time of the fighting against Hitler's occupying forces, the function carried out[,] the army rank, in which divisions she served, groups, postings, under whose command, directions, taking part in fighting and other actions, in which places, the name of the camp etc. any changes and periods of time[)].

After the liquidation of the Miechów ghetto in 1943 with the help of Mrs. Jadwiga Kozłowska-Borkowska I fled to Cracow and was hidden by her from the first moment until mid-January 1945 i.e. until liberation. With satisfaction, I wish to underline, that Mrs. Jadwiga Kozłowska-Borkowska putting her life and those of her family at risk hid in this time not only myself but from time to time several [other] Jewish men and women. She did this completely without gain, she didn't take any money either from me or from the others hiding at her place, for the flat, food or even getting us clothes, or for help in buying medications for us, in all that time.
Mrs J. Kozłowska raised our spirits, which at that time had big significance. Mrs. Jadwiga Kozłowska-Borkowska drove from her own initiative and at her own cost to Miechów to save my father of blessed memory from the transport to Oświęcim, insodoing, seriously risking her life, unfortunately she did not manage to accomplish this becuase she found out my father had already been loaded onto a wagon. At that opportunity she brought me valuable things of mine from Miechów, which she returned to me in full after the occupation, not even charging me for the costs she incurred.

It is known to me, that Mrs. Jadwiga Kozłowska-Borkowska was taking care of delivering food and medication during the occupation in the territory of Cracow and Wieliczka to many Jewish families in hiding. I underline, that with her deeds and behaviour, she actively resisted the German action of exterminating the Jews. Her dedication to saving Jews[,] giving material and moral support had no limits.

should be ignored as it is standard wording from the form that would be crossed out depending on the service of the person giving the statement. This became clear when I saw the other document you uploaded, which is the original of this one.
