These are the results of the medical examination from the pre-examination in Canada when Annie Rosenbloom crossed the border into the US in June, 1919.
It appears to say she has some medical condition. She died 4 years later. Can anyone determine what it says?

5/6/2020 8:10 PM declines to have ____ examined [last word uncertain].
[ ____ looks like "_calf". the leading _ looks more like it's an 's' than a leading flourish]. final letter might be something other than 'f' but unlikely a 'p'. penultimate letter might not be an 'l'
5/7/2020 9:03 AM the preprinted choice is between "free from disease" or "afflicted with". The examiner has failed to circle either one, and has written "declines to have scalp examined". Presumably for lice or whatever. So they are unable to certify either way.
5/8/2020 12:35 AM It says Declined to be examined but he passed her. Chances are it was something that embarrassed her like lice, ringworm, baldness or if she was Orthodox she would have been wearing a wig.In any case I don't believe it contributed to her death 4 years later. Look at the death certificate to see cause of death.
