On this the twenty-eighth day of the month of December in the year of eighteen forty, at the hour of two in the afternoon, before me, Filip Etgeno, the Deputy Civil Registrar in the Free City of Cracow established for the observant Jews, running his office in the Second Commune at number 171, personally appeared observant Jew Symche Hertzog, the brother of child’s father, thirty-one years of age, residing in the Sixth Commune at number 20, and presented to us a child of the male sex, born in the eighteenth day of December of the current year, at the hour of three in the morning, in the house of residence of his parents, declaring that he was born from the observant Jews Selig Hertzog, a trader, thirty-six years of age, and Ella Jachet nee Frener, thirty-four years of age, the married couple residing in the Sixth Commune at number 183, to whom, in accordance with the wish of his parents, the name had been given Jozef.
