Family Surname: Izakovics

This image is a note added on the birth record of Haem Izakovics, born in 1912 in Aknaszlatina, Hungaria. The note is written in Cyrillic, probably Russian with Ukrainian words.

I don't have a better definition for this image.

I can only decipher the name: "Haem", the date: "2 grudnia (December in Ukrainian) 1967" followed by 2.XII.1967, and "ЗАГС" (the civil registration office in Russian).

I suppose that it provides the date of death, but I would like confirmation if someone can actually read this!

Thanks, Nicolas

2/12/2020 6:10 PM No, it's name change. When you see names written within inverted commas, you can be sure that the name itself is the object of the note.
From Haem Li[?]... to Haem
2/13/2020 11:37 AM I didn't succeed to read all the text, but I'm sure it is not the "name change" but the "correction of name".
The copy is diificult to read.
