This is a page from my grandmother's autograph book. Her name was Roza Simanowski. I would really appreciate a word-for-word transcription and translation.

10/1/2017 7:08 PM Perhaps unusual for a European text, this is in fact in Hebrew and begins with "a memento..." You may wish to adjust the language tag in the introduction.

10/3/2017 5:28 AM You also asked for a transcription, which I am able to do thanks to the hints that are now available from the translation that you received. So I will write this in Hebrew letters, and then transliterate:

Transcription:
לזכרון נצח!
פרח. ? פרח ואף תבל.
באיזה מקום שתהי אותי אל תשכח.

Transliteration:
l'zichron netzach
p'rach. ? p'rach v'af tevel
b'aizeh makom sh't'hi, otti al tishkach.
