This is an annotation, in the name section, of a birth record from 1911. Since the annotation was during the Russian occupation, it is possible that this annotation is in Russian, not Polish. What does it mean?

5/14/2016 4:07 AM It's Ukrainian. Certificate issued, Feb 28, 1940. I think the bottom line is the name of the the clerk, Tileman or Tisleman
