Can you translate?

This is one of four images. This one has the birth entries from my great grandfather Hersch Jaged and his brothers, Aaron and Isaak. The other attachments I'm posting are birth and death entries of Michel and Moses, both of whom died as infants. I'm also posting the chart of the summary findings.

Can you translate from the images?

These are my great grandfather, his siblings, and their parents (my great, great grandparents).
It's from Narajow, then Austria-Hungary, then Poland, then USSR, now Ukraine. The column headings are in German and Polish, but I think the handwritten info is Polish.

Many, many thanks in advance.
Rick

2/5/2014 2:04 PM The entries are written in German, not Polish. They are hard to read because the resolution of the scans is too low. If possible, scan again with 200 or 300 dpi and upload again.
