Herr Medizinalpraktikant Dr Nathan Roos
hat im Laufe des Frühjahres zu zwei Erstimpfungs-
+ zwei Wiederimpfungsterminen + den
entsprechenden Nachschauterminen angemahnt.

Karlsruhe, 23 June 1914
During the spring, the medical intern Dr Nathan Roos has issued reminders for two appointments for initial vaccinations and two appointments for booster vacctinations and the corrresponding followup sessions.

Note:
It is impossible to interpret this document without knowing how the vaccinatin campaigns were organized at the time, an information that I do not have. The word "Termin" can mean "appointment" or any other time within a schedule, such as a deadline. I would soppose that vaccinaton clinics were run at specific dates and that vaccination (presumably against smallpox) was compulsory, so the citizens had to show up at these dates. The word "angemahnt" (perfect participle of "anmahnen", to admonish or remind) is not written clearly.
