I would appreciate help with a translation of my G.G.Grandmothers death certificate,I believe it is written in Polish but I am not sure.The listing is on the fourth row,Her name is Rivka Kleinmann married to Menasha Fischthal from Czortkow.Why would her death certificate list her under her married name?
Please tell me what the column heading are and any thing else written you can translate.

5/26/2010 7:25 PM My husband is Arnold Fischthal, son of Hersh Tzvi Fischthal from Czortkow. I can't read the document-- some of it looks like it's in German. Certainly, "Fischthal" is spelled the German way. I note the date 1897, which is when Czortkow was part of the Austria-Hungary Empire, which would explain the German.
