6/11/2009 8 Adar in the year 5689 by the abbreviated era

5689 was a leap year but there is no indication on
the stone if she died on First Adar or Second Adar

Which is 15 or 18 Fab 1929.
The difference between 5 and 8 is whether one line is connected to the other one or not. well -it is not connected -and so looks more like 5 but much closer than other times that letter appears on the tombstone and so it may also be 8. the 8th letter does not appear another time so I can't compare.

The last letter is not clear but I think this is correct.
6/12/2009 the date is the 8th of adar 5689 [18 feb 1929].

also note that her father and husband were both rabbis.
6/12/2009 8 Adar [5]689 (18 February 1929). The add letter after the year is a combination of a Lamed and Kuf and is the acronym Leperek Katan that it is a "short version of the year, missing the digit 5 (689 and not the full 5689).

I read the date as the 5th of Adar, 5689
which would be Feb. 15, 1929

Without further information there's no way to tell which Adar it was.
