Information Picture Question

Possible location of this tombstone. It is most likely the grave of my great-grandmother Chaya Leah Chrzan, who died in 1909. I have an older picture of this tombstone indicating it is in Warsaw. I believe this is a postwar picture noting the delapidated condition. I have been unable to find it in a present-day cemetery. Suggestions as to where to look would be welcomed

5/15/2009 The name is just about illegible on this stone. It is hidden by the grasses and rubbed out. The top part says Here is buried an elderly, respected, G-d fearing woman... I think her first name was Chaya but it is impossible to know for sure. Her father's name seems to be Yaakov/Jacob and that's all I can see.

Resolution is not vegood, hard to read , this is what I have
" an old humble woman
important and with god's fear ,
donated to charity all her life
Mrs XX(2 letters) Leah (could be the common name Leah with an xcronym before it)
daughter of Jacob (3-4 more letters probably the surname + starts with B)
2 more lines -can't read.

5/19/2009 THE TOMB IS OF AN OLD WOMAN. THE NAME IS UNCLEAR. SHE IS THE DAUGHTER OF R' YAAKOV (THE FAMILY NAME IS UNCLEAR)

Peformed charity [?]
Mrs.(?) ?-A-K-? (could be zaks)
Daughter of Reb Yaakov(?)
