This is a death record.

In column order:

1. Record #: 92

2. Place and date of death certificate and name of person viewing the body: Kapuscince, 19 November 1902, #31. Jozef REGUORZ?

3. The death: 12 noon, 19 November 1902, Kapuscince, house #114.

4. The funeral: 21 November 1902 in Ułaszkowce.

5. The deceased: Eige Ruchel ROSENSTOK f (false*) MANN, married according to Mosaic rites to Boruch MANN in Kapuscince. Daughter of Mendel and Jenta ROZENSTOK from ? (I can't make out the place name). Place of residence for the deceased resident is Kapuscince, house #114.

6. Gender: Female

7. Age: 50

8. Marital status: single (as opposed to married or widowed)

9. Cause of death: Consumption

*This is the explanation for the f (false):

"False" (often abbreviated f.) implies usage and means an individual might be using the father's surname but officially should be using the mother's surname due to lack of the required civil marriage. Example: Brown f. Cohen means the person used Cohen (father's surname) but Brown is the legal name (mother's surname).
