I'd appreciate help translating all hand-written entries in this one Polish record related to Herzyl Schulmann's birth in Podhajce in 1893, taken from p.80 and p. 81 of a metrical book for Galicia on file at AGAD.

This upload shows the right side of ledger. The left side is in my previous upload. I'm eager to know the exact dates in Col. 1 and 2 (and whether they are being given in the Gregorian or Julian calendar, if known) on the left side of the page.

Also very eager to decipher ALL the info in Col. 7 on the right side of the page, which names the baby's parents and maternal grandparents and has a hard-to-read place name other than Podhajce that starts with a B perhaps. It may also divulge an occupation for the boy's dad.

Col 12 also has shorthand notes that might benefit from translation from Polish. Thank you in advance for providing this wonderful form of assistance!
