The translation is as follows:

To remember the soul of Tzvi Hersh, Ben/(son of) Reb Avrohom, who died on the 5th day of the Hebrew month of Kislev (followed by the Hebrew year). May his soul be bound up in the bonds of life."

The word "Reb", which precedes the father's single given name of Avrohom, is used as a title of respect. It does NOT imply rabbi.

12/2/2020 4:22 PM The full Hebrew date is the 5th (or possibly the 8th) of Kislev, 5713.

It is difficult to say for sure whether the date is the 5th or the 8th. Here are the corresponding secular dates for both:

The 5th of Kislev, 5713, began at sunset on November 22nd, 1952, and ended at sunset on the 23rd.

The 8th of Kislev, 5713, began at sunset on November 25th, 1952, and ended at sunset on the 26th.
