I would like to know if this is the Israel Bober who was my great-uncle from Poland. He was originally from Wysokie Mazowieckie (Visoko Mazovist), Poland, but I have an old studio portrait of him with taken in a photo studio identified in Zamarew (Zambrow?). I would appreciate a loose translation of this Holocaust document.

8/5/2014 6:08 AM The name of the place of birth in Hebrew letters looks like Kotzk (Kock according to Wikipedia)but I'm not sure this is what the Latin letters say. You can check if Kotzk is located near his known place of birth.
8/6/2014 3:03 AM i agree with the second responder: the place of birth is KOTZK.
