8/19/2014 7:14 AM The names Jakub i (and) Josef are underlined on the top line of this page. The word after the names is split between the first two lines. It might be a variation of the word bliźniaki or bliźnięta (twins).

Before me, a Civil Registrar of Belchatow Commune in Piotrkow County located in Kalisz Voivodeship, on this, the twenty-ninth day of the month of January in the year of one thousand eight hundred and twenty-one at the hour of three in the afternoon, personally appeared orthodox Jew Maier Zelwer, a baker by profession, forty-three years of age, residing in Belchatow, and showed us a baby of sex male who had been born in his house at number seventeen the day before at the hour of ten in the evening declaring that he was the begotten son of him and his spouse Debora nee Birnbaum, thirty-five years of age, their marriage being a first marriage for both of them, and that it was his wish to give him the double name Jakub Jozef. It was in the presence of orthodox Jews Mortka Ways, forty-six years of age, a trader in tobacco and snuff, and Michal Birnbaum, sixty-four years of age, a merchant and the same time a trader in cloth, both of them residing in the Town of Belchatow, that the above statement was made and the baby was showed to us. At last, the actual act of birth was read aloud to the appearers and subsequently signed by the parent, both witnesses and myself.
