Birth record of Chaje daughter of Todrys Ast and Rachel Kilaffe

There is Polish at the bottom of Col 10. Please do a direct translation of any Polish on this page.

Under her name I think it shows her made legitamate.
Please directly translate the unknown words or columns listed on the document. I can send you the scan of the original page if needed.

Thank you.

6/3/2012 9:30 PM Column 2 describes the place she was born.
Column 3 is the place where the circumcision, of a boy, or the announcement of her name, for a girl, took place. Religious families formally announce a girl's name in the synagogue, a few days after her birth; This explains the different house numbers.

6/3/2012 9:46 PM Column 7 says Rachel Kilafe of Brzezan, daughter of the married Chaim and Sara Kitafe of Pomorzany. The first of column 12 is I think the father acknowledging the birth of his daughter. And the second half says "zeuge" which is the witness with the name Hoenoch Hoenig
6/6/2012 7:01 AM This is a standard Galician birth record. You will find the translation of the headings here:
http://www.texsys.com/HoeflerGottesfeldPapers/BirthHeader.htm
Transcription:
1. 56
2. 10 [you will find the month and the year on previous records] [house number] 113
3. 13 [you will find the month and the year on previous records] [house number] 24
4. Chaje [female]
5. illegitimate
6. Todrys Ast, Schänker [= Innkeeper] in Brzezan
7. Rachel Kilafe in Brzezan, Tochter der Eheleute [= daughter of the married couple] Chaim und Sara Kilafe in Pomorzany
10. [This is the name of the midwife. There are two words in latin: per me (= by me).]
12. Ich bekenne mich als Vater des Kinde u. willige in meine Eintragung als sochen ein. [= I declare to be the father of the child and I agree to be registered as such]. Tovrose[??], als Zeuge (= as witness) Hönoch Hönig

There is an entry in Polish at the bottom of cloumn 10. I cannot read Polish, but I assume that this is about the legitimization of the child, with the shelf mark of the book where this is documented.
