All this occured in the Town of Lomza on the twenty-third day of December in the year of one thousand eight hundred and sixty-three at the hour of nine in the morning. Appeared personally Orthodox Jew Daniel Herszkowicz Kawkiewicz, the teacher* residing in Lomza, forty-eight years of age, accompanied by witnesses, namely Orthodox Jews Eliasz Rozenblum, sixty-three years of age, and Abram Rozenberg, the teacher forty years of age, both of them residing in Lomza, and showed us a female baby who had been born in Lomza on the seventeenth day of August in the year of one thousand eight hundred and fifty-five at the hour of twelve at noon to his spouse Raszka the daughter of Icek, forty years of age, to whom the name had been given Guta. It had been the father’s negligence {in reporting the birth} which was the reason why it was only that day that the actual certificate was drawn up, after which it was read aloud to the father and the witnesses and subsequently signed by the father and the witness Eliasz Rozenblum. The other witness was unable to write.

NOTES: *Probably “bakalarz”; needs to be verified though as the expression indicating his occupation is rather illegible. This is what I was able to make out.
