8/21/2013 9:46 PM If I'm understanding your question correctly, this document is an identity card for the Republic of Poland. Corrections you may find interesting:
The parent's Polish names are Moszek and Jachweta (maiden name? Ejzenberg)
The street name is from the Italian revolutionary Garibaldi.
