99. Stryjków [actually Stryków], year 1811, 31. December, at 2 PM. In front of us, the parish priest of the Stryjków parish, civil officer of the Stryjków township, of the Brzeziny District, of the Warsaw Department, appeared Josek Ickowicz Waytz, of mosaic faith, haberdasher,
46 years old, living in Stryjków, and showed us a male child born in the house number 26, on 29 December at 2 PM. He declared that [the child] is born of him and his wife Roza, born Israel, 40 years old, and that he wishes to name the child Jakub.
After having declared the above and having shown the child, in the presence of
Icek Jakubowicz Brennman [? Brezinman?], taylor, 40 yrs.
Samuel Israel Henrich [?], haberdasher, 40 yrs.
of mosaic faith, living in Stryjków.
