I think this document certifies that [hard to read given name] Apter has been listed in the conscription sheet of his widowed mother Lea Apter and that he was born in 1876. It is signed by the chairman of the Jewish community (kehilah) of Tarnopol. The conscription was done by property, and because the house belonged to his widowed mother, he was listed as her son. The involvement of the kehilah is unclear, but the kehilah also had the authority to keep the Jewish birth registers, so it might also be responsible for the conscription. Maybe some other genner knows more about how the conscription was organized in 1894 in Austria.

The first name looks like Chaim to me. Chaim Apter.
