(The following) took place in the town of Lukov on May 24th 1894,at 4.00 p.m. Appeared(before us) Shlama Mendel Vizel, 35 years old, tradesman, living in the town of Lukov, in presence of the witnesses Yankel Rozenshteyn, “feldsher” (an assistant to the chief doctor in the hospital), aged 48, and of Shmul Feldentsher, tradesman, aged 30, both living in the town of Lukov, and presented to us a female child, declaring that she was born in the town of Lukov on the 24th of March of the present year, at 2.00 p.m., from his legitimate wife Khava,born Gardovnik, aged 32. To this child was given the name of Rukhlya. The father explained that being absent (when the child was born), he has been late for declaring the child. The present act was read to the declarer and to the witnesses, and then undersigned by us and by the witnesses. The child’s father is illiterate (cannot sign his name).
