This is the 1896 death record for my great grandfather Szaje Kohl. I would like a translation of the entire entry for him, but I'm particularly interested in the names of his parents, his occupation and cause of death (columns 5 and 9). Thanks for your help.

On  Response
5/1/2013 6:40 AM There are two persons listed within the red lines you drew.
One is your great grandfather Szaje Kohl. The other one is a stillborn (unnamed) baby boy to Leibisch and Freida Apfelbaum.
Their information is separated by a very fine, horizontal pencil line.

Szaje Kohl died in Hakalo.... (?) May 25, 1896, at 11 o'clock, and the death certificate was verified by a S.Korsky (?) on May 26 at 18 o'clock.
Szaje Kohl was burried in Zalosec on May 27, 1896.
He was 69 years old, was married, and died of edema - "Wassersucht".

- column 5 states only your great grandfather's name, not his occupation, and not his parents' names.
- column 9 notes that the death certificate was mailed to the relevant registry office to have the death registered there:
"den Todesschein an das betreffende Mat(rikulations) Amt zur Inmat(rikulation) geschickt"

Maybe others can help identify the correct names of the cities?
Sincerely,
CK
