On  Response
3/6/2013 10:02 PM This letter was addressed by Leyb and Sarah Rivka Walraich to his uncle in America (your grandfather).
The sender's address on the envelope indicates that he is active on the Jewish Committee in the town of Piotrkow.
In the letter he thanks his uncle for the parcel and says he makes a living for himself and his wife from the contents of the parcels he gets from America. He also mentions an "uncle Moritz" from America who has sent him a parcel.
The added note is from a certain Bernstein whom Leyb visited following his aunt's (your grandfather's wife) request to find out what he knows about her brother, Yudel, and his son, because Bernstein hid together with them in the woods where they all tried to survive as partisans. It seems Yudel and his son got lost in the woods and then they lost track of each other.
This Bernstein is sending the added note you posted, asking your grandfather to find out the whereabouts of his mother Esther's brother in America, his uncle Simtse Gebiroivitch, because he doesn't remember in which town of America he lives.

Hope this has helped you.
3/7/2013 1:18 AM Forgot a few things in the summary above: the full name of the town is Piotrków Trybunalski (note the "Tryb" at the end of the sender's address); Leyb tells his uncle, you grandfather, that his wife was admitted to hospital in Otwock (pronounced "ot-votsk") with an unspecified chronic ailment.

You can find more info on the town in the translated Yizkor-book here:
http://www.jewishgen.org/Yizkor/Piotrkow/Piotrkow.html

and on the web page of this research project here: http://www.holocaustresearchproject.org/nazioccupation/piotrkow.html

Hope this helps some more.
