HI THERE, CAN ANY BODY TRANSLATE THE HEBREW ON TOP OF THE GRAVE, I KNOW THIS ONE MAY BE DIFFICULT, DUE TO FALLEN OUT LETTERS.

Father's name has only one fully legible character.
11/12/2012 7:37 AM Based on the holes for the letters, the father's name could be "Moshe".
11/13/2012 6:56 AM I agree with Moshe - mem shin he. The right hand part of the letter mem has completely disappeared, but the left hand part is sitll visible. Note that it is slightly slanted, as opposed to other letters where the downstrokes are vertical. Compare the position of the bolts to those on the mem at the beginning of the line.
