What is the direct translation of the headers? Please answer the questions I wrote on the document. Thank you.

6/22/2012 6:01 PM	Doctor's name is Uranowicz
6/22/2012 7:24 PM	The death occurred on the 28th, the medical examiner's report was filed the 29th by Dr. Uranowicz, and the burial was on the 30th. Malka Ast, ritualle Gattin des Nuchim Ast, Mehlhändler in Brzezany. Malka Ast, ritual wive (meaning married religiously witout entry into the civil registers) of Muchim Ast, flour merchant in Brzezany. House number 433. (both for the death and the residense of the deceased). Female, 67 years old, died of pulmonary emphysema. There is never a cemetery mentioned in this kind of records.
