Second half of my grandmother's departure document (1st half also downloaded), in very poor condition. I'd like a translation of the categories at the top. I know some, like the date and name, but not most of the others. Her name was Estera Malka Abromowicz, but I'd like any help with just about everything else.

I am translating this from the German, as my knowledge of Polish is zero.

The column at the tear requires the name, etc of the father of the child whose birth is being registered.

The next column is for fore and surnames of the mother, her status, town where she lives, and the names of her parents.

The follows names of witnesses (to the registration, not necessarily the birth).

The next column is the name of the "mohel" the person doing the circumcision - obviously blank for a female

Then the name of the midwife - also blank

I cannot make out the next narrow column, but I think it refers to siblings.

The final column is Remarks or Comments.
