Third line from bottom in Hebrew: Nolad...Adar = was born in Adar (Hebrew month)

Second line from bottom in Hebrew: Kislev = (died) in the month of Hebrew month of Kislev.

He was born on the 2nd day of Adar, 5614 (March 1/2, 1854)
And he died on the 27th of Kislev, 5682 (December 27/28, 1921)

May His Soul Be Bound Up in the Bundle of Life

The small letters may be Pln; Poland.
It is unusual to have so much detail on stone unless the person was a true Rabbi, I believe.
