Document 5/5 received from USHM. Need translation and connection to the other 4 documents. Mainly, what is the origin of this document and would it mean that Zigmund was still alive in 1948 ?

3/20/2008 I received similar document from USHMM - in fact I received about 10 of them, all about one person, all giving one more detail than the one before. I think they were all from different documents. Did you receive any others? The first couple I received didn't give a date of death, but the others did. Do you know if this person died during the war?

This is another response from Mauthausen records on Sept. 12, 1949.
It doesn't have any additional information, only the name and file number.
I can't tell you what this card means, I can only help with the translation. Your relative had untreated TB and was transferred from Auschwitz to Mauthausen to Gusen just before the end of the war. Was he alive in 1949? Your guess is as good as mine.
I hope that this is of some help. Please let me know how you received this information. I too would like to know the whereabouts of some relatives who were sent to the Lodz ghetto and we are not sure where they died.

this card, dated sept 12, 1949 indicates the date the card was made and gives Zygmund's file #O CC 15/145. Were you at USHMM you would then look at what that stands for, pull that file, and look on list 15 probably page 145 [not a 100% on this part] to see what kind of list he is on and what it says
this was probably an inquiry card generated when some person or organization was seeking information...ie. some agency was making a list of Krakow born prisoners in Mauthausen or some relative wrote ITS asking what they had on Zygmund. IT is NOT an indication of either death or survival.
